I acknowledge and pledge with my full unconditional consent to authorize the subscriber Commercial Bank of Kuwait (K.S.C.P) to inquire about all my data and information from the Central Risk automated system of the Kuwait Credit Information Network Company. I also declare my unconditional consent that the subscriber Commercial Bank of Kuwait (K.S.C.P) provides all my credit and personal data and information stored in its systems to the Credit Information Network Company under the contract concluded between them for the exchange of credit information and business rules established by the Central Bank of Kuwait, including my personal basic information registered at the Public Authority for Civil Information and all my credit data and information with analytical and detailed information, including my credit rating indicators for all types of transactions and credit facilities of all kinds and names, whether for consumer or housing purposes, credit cards or any other related purposes, guarantees of all forms and all credit facilities granted by way of sale in any form of ownership transfer by installments for goods and services in accordance with the provisions of Law No. (9) of 2019 on the organization of the exchange of credit information and its executive regulations and the regulations and controls issued by regulatory authorities and the subsequent amendments thereto, without any responsibility on the Credit Information Network Company for any consequence of the incorrectness or inaccuracy of the personal or credit information contained in the credit report, or in case of violation or noncompliance with the Law of the Civil Information Authority Law No. (32) of 1982 or the provisions of Law No. (9) of 2019 governing the exchange of credit information and all related laws and any amendments to those laws, decisions and controls issued or will be issued in implementation thereof without objection or reservation. This is an acknowledgment and a consent from my part for the abovementioned information.

أقر وأتعهد بموافقتي الكاملة غير المشروطة على تفويض المشترك / البنك التجاري الكويتي (ش.م.ك.ع) بالإستعلام عن جميع البيانات والمعلومات الخاصة بي من النظام اآللي لمركزية المخاطر لشركة شبكة الكويتللمعلومات الإئتمانية ، كما أقر بموافقتي غير المشروطة على أن يقوم المشترك )البنك التجاري الكويتي (ش.م.ك.ع)) بتزويد كافة البيانات والمعلومات الإئتمانية والشخصية الخاصة بي والمسجلة لديه في انظمته إلى شركة شبكة المعلومات االئتمانية بموجب العقد المبرم فيما بينهما والخاصة بتبادل المعلومات االئتمانية وقواعد العمل الموضوعة من قبل بنك الكويت المركزي ، والتي تشمل بياناتي الأساسية الشخصية المسجلة لدى الهيئة العامة للمعلومات المدنية وجميع البيانات والمعلومات االئتمانية الخاصة بي وما تتضمنه من معلومات تحليلية وتفصيلية بما في ذلك مؤشرات التصنيف الإئتماني الخاصة بي وذلك عن جميع أنواع المعامالت والتسهيالت الإئتمانية بجميع أنواعها ومسمياتها سواءا كانت لأغراض إستهالكية أو إسكانية أو البطاقات االئتمانية أو أية أغراض أخرى ذات صلة ، والكفالات بكافة أشكالها، وجميع التسهيالت الإئتمانية الممنوحة بطريق البيع بأي شكل من أشكال نقل الملكية بالتقسيط للسلع والخدمات سواءا كانت من المشترك المفوض له بالإستعلام أو من أية جهات أخرى المنصوص عليهم في أحكام القانون رقم )9 ( لسنة 2019 في شأن تنظيم تبادل المعلومات االئتمانية والئحته التنفيذية والنظم والضوابط الصادرة من الجهات الرقابية له والتعديالت الالحقة عليهم ، وذلك دون وقوع أدنى مسئولية على شركة شبكة المعلومات الإئتمانيةلما يترتب عليه من عدم صحة البيانات والمعلومات الواردة في تقرير المعلومات الإئتمانية الخاص بي في حال اذا ما كانت مخالفة أو غير متوافقة مع قانون نظام هيئة المعلومات المدنية رقم (32) لسنة 1982 أو أحكام القانون رقم (9) لسنة 2019 في شأن تنظيم تبادل المعلومات الإئتمانية وجميع القوانين ذات الصلة وأية تعديالت تطرأ على تلك القوانين والقرارات والضوابط الصادرة أو التي ستصدر تنفيذا لهم دون إعتراض أو تحفظ وهذا إقرار وموافقة مني بذلك على ما ورد أعاله .
